To support your claim, we’ll need to see either a Fit Note from your doctor or a report from your health practitioner or a hospital.
The document should include the first date you were unable to work, along with how long they expect you’ll be unable to carry out your usual duties.
Please make sure the document includes a stamp from the clinic or hospital.
