When you make this call Zurich will share their contact details with you. Then all supporting documentation should be shared to this contact. However, if you have not called Zurich Assist and still wish to make a claim then please complete the in-app claim form and attach your documentation where indicated.

- A medical certificate from the medical practitioner to confirm the extent of the injury and treatment given including
hospital admission/discharge.
